From Fabrizio and me:
- add support for native recursive mutexes (pthread and Win32) and remove
  not satisfactory generic recursive implementation.
- remove support for older pthread versions (PthreadDraftVersion < 10),
  all modern systems support version 10 now.

Set property svn:eol-style LF on all source and Makefiles. This should avoid
problems with Win32 line endings ending up in the repository. All MS tools
support LF eols fine.

remove a bunch of $Name$ tags that were missed due to a problem in the
initial svn patch up script. Also add svn:keywords Id to all files not
