[ROOT] Anyone with example forms?

Hi - I'd like to build a GUI interface for my code.  It would consist of
some buttons that would run commands, as well as a form-type interface.
The form would display the values of certain variables in memory, and also
have boxes next to each value that allow the user to enter new values.

I've seen an example for a GUI with only buttons.  I was wondering if
anyone out there has example code for a GUI with the additional
form functionality I'm looking for that I could easily customize for my
own purposes.
